When you are trying to pass a unique identifier through a web form you need to add a custom field called fbAFFID. In the URL you will then add a string that looks like this...&fbAFFID=YourUniqueIDHere. This will pass the unique identifier.
Warning - Your form Name cannot have any special characters like - or ' .
